What the Masseter Is

The masseter is the thick muscle you can feel bulge at the angle of your jaw when you clench your teeth. It is one of the strongest muscles in the body relative to its size, and it works every time you chew.

In some people it is overdeveloped. Chronic clenching, night-time grinding, gum chewing, and simple genetics can all enlarge it. The result is two separate problems that share one cause:

• Function: jaw tension, morning jaw soreness, tension-type headaches around the temples, and worn or chipped teeth from grinding.
• Shape: a wider, squarer lower face than your bone structure alone would give you.

Botox addresses both, because both come from the same overactive muscle.

How Masseter Botox Works

Botox is a neuromodulator. Injected into the masseter, it partially blocks the nerve signal that tells the muscle to contract. The muscle cannot clench as forcefully, so the grinding and tension ease. Over the following weeks, a muscle that no longer works at full strength gradually loses some bulk, and the jawline narrows.

You still chew normally. The dose is chosen to take the excess off the top, not to disable the muscle.

Typical Dose

The masseter is a big muscle, so it needs more units than the forehead or frown lines. A typical dose is 30-50 units per side, adjusted to your muscle size and how much of the effect you want. Timeline: When You Will Notice It

Two things happen on two different clocks.

Tension relief follows the usual Botox timeline: onset at day 3-5, full effect by day 14. Many patients notice they wake up without a sore jaw within the first two weeks.

Slimming develops more slowly. The muscle has to shrink from reduced use, and that takes weeks rather than days. Expect the shape change to become visible gradually after the tension relief, and judge your result at your follow-up rather than in the first fortnight.

How Long It Lasts

Masseter Botox often outlasts upper-face Botox. Where forehead and frown-line treatment typically holds for 3-4 months, masseter results often last 6 months or more. The muscle is large and the effect on its bulk is slow to reverse. Patients who treat consistently frequently find they can stretch the interval between visits over time. Is It Covered by Insurance?

Cosmetic Botox is not covered by OHIP or private plans. Some extended health plans partially cover Botox for specific therapeutic indications. Coverage depends entirely on your plan, so call your insurer directly and ask about your particular situation before your appointment. We provide a receipt for any claim.

Who Is a Good Candidate

Masseter Botox suits most healthy adults who:

• Clench or grind their teeth, especially at night
• Have jaw tension or soreness on waking
• Feel their lower face is wider than they would like, and the width is muscle rather than bone
• Want a non-surgical option and understand the result builds gradually

We will not treat you if you are pregnant or breastfeeding, have a neuromuscular condition such as myasthenia gravis, have an active skin infection in the area, or have a known allergy to botulinum toxin or albumin. If your jaw width is mostly bone rather than muscle, Botox will do little for the shape, and I will tell you that at your consultation rather than treat you anyway.

What to Expect at the Appointment

Masseter injection is quick. After a consultation and assessment (I will ask you to clench so I can feel the muscle borders), the injections themselves take a few minutes per side with a very fine needle. Most patients describe a brief pinch. There is no real downtime. Follow the standard Botox aftercare: no lying flat for 4 hours, no heavy exercise or heat for 24 hours.

Some patients notice their chewing feels slightly weaker for a few weeks, particularly on very tough or chewy food. That is expected and settles.
