Botox Aftercare: Exactly What to Do (and Avoid) in the First 24 Hours

Short answer: for 4 hours after Botox, stay upright and do not rub the treated area. For 24 hours, skip heavy exercise, heat, alcohol, facials and aggressive skincare. Then live normally. Results start at day 3-5 and are final at day 14.

Botox aftercare is simpler than filler aftercare, but the first few hours still matter. Here is exactly what I tell my patients when they leave the chair.

Why Aftercare Matters for Botox

Botox is injected into a specific muscle in a small volume. In the first few hours it binds to the nerve endings in that muscle. Until it has bound, anything that pushes it around, whether pressure, heat or vigorous blood flow, can theoretically move a little of it toward a muscle you did not intend to treat. That is the whole logic behind the rules below. After roughly 4 hours the product is bound and the risk is gone.

The First 4 Hours

Stay upright. Do not lie flat, nap, or bend over for long stretches. Sitting or standing is fine.

Do not touch, rub or massage the area. No face washing with pressure, no cleansing brushes, no resting your chin or forehead on your hand.

Use your muscles gently. In the first hour, normal facial expressions (frowning, raising your brows, smiling) are actually helpful. They help the product distribute within the treated muscle. You do not need to exaggerate them.

No hats, headbands or helmets that press on a treated forehead. Loose is fine; tight is not.

The First 24 Hours

Avoid heavy exercise and sweating. Raised blood pressure and circulation are the concern. A gentle walk is fine. The gym, running, hot yoga and anything that leaves you flushed can wait until tomorrow.

Avoid heat. Saunas, steam rooms, hot tubs, and very hot showers on the face.

Avoid alcohol for the rest of the day. Alcohol dilates blood vessels and increases the chance of bruising.

Skip facials and facial massage. This includes professional treatments and at-home devices like massage rollers or gua sha.

Pause aggressive skincare on the treated area: retinol, glycolic or salicylic acids, exfoliating scrubs, and microneedling devices. Gentle cleanser and moisturizer are fine.

Makeup: wait until the following day, then apply gently. Injection sites need a few hours to close; giving them a full day is the tidy approach.

What Is Normal

You may notice any of these in the first hour or two:

  • Small raised bumps at each injection site. These are just the liquid volume and flatten within about an hour.
  • Mild redness at the injection points. Usually gone within 30 minutes.
  • A slight ache or heaviness in the treated area. Mild and short-lived.

In the days that follow:

  • Minor bruising happens in roughly 5-10% of patients and fades in 3-5 days. A gentle wrapped cold compress can help after the first 4 hours (no pressure on the area before then).
  • A mild headache in the first day or two, especially after a first treatment. Avoid blood-thinning painkillers like ibuprofen or aspirin if you can, since they increase bruising.

What Is Not Normal: When to Call

Complications from Botox are rare, but I want to hear from you immediately if you notice:

  • A drooping eyelid or brow that appears in the days after treatment
  • Vision changes or double vision
  • Difficulty swallowing, speaking or breathing
  • A severe headache that is out of proportion to anything you have had before

Call 613-869-3269. If it is after hours and symptoms are severe, go to your nearest emergency department. I would much rather take a call about something that turns out to be nothing.

Days 2 to 14: Waiting for the Result

Botox does not work instantly.

  • Day 0-2: no visible change. This is normal.
  • Day 3-5: lines begin to soften with movement.
  • Day 7-14: the effect keeps building.
  • Day 14: your final result, and the right time for your follow-up.

Aftercare Myths

"I have to keep making faces for hours." No. Gentle, normal expressions in the first hour are enough.

"Massaging my face later will make it wear off faster." Once Botox is bound (about 4 hours), massage does not move it or shorten its duration.

"I can't wash my face tonight." You can, gently, with your hands and no pressure, after the first 4 hours.

"I have to sleep sitting up." No. By bedtime you are well past the 4-hour window. Sleep however you like.

Before Your Next Treatment

A little preparation reduces bruising at your next visit: for 24-48 hours beforehand, avoid alcohol, fish oil, vitamin E, and blood-thinning painkillers like ibuprofen or aspirin unless you take them on a doctor's advice. Arrive with clean skin and tell us about any new medications.
