Lip Flip vs. Lip Filler: Which Should You Try First?
Short answer: a lip flip is a few units of Botox that relax the upper lip so it rolls slightly outward. It shows more of the lip you already have and lasts 6-8 weeks. Lip filler is hyaluronic acid gel that adds real volume and shape and lasts 6-12 months. If you want a subtle change with zero downtime, start with a flip. If you want visible fullness, you need filler.
Patients often book a "lip" consultation without knowing these are two completely different treatments. Here is how I help them decide.
What a Lip Flip Does
A lip flip uses Botox, not filler. I place 4-6 units into the muscle that circles the mouth, just above the upper lip border. The muscle relaxes slightly, the upper lip stops curling under when you smile, and a little more of the pink lip shows.
- What changes: the upper lip rolls outward and appears slightly fuller. Nothing is added.
- Onset: a few days, full effect around two weeks, like all Botox.
- Duration: about 6-8 weeks. This is much shorter than Botox elsewhere because the dose is tiny and the muscle is used constantly.
- Downtime: none. You can go back to your day.
- Feel: for the first week or two, some patients notice that sipping through a straw or whistling feels slightly different.
What Lip Filler Does
Lip filler is hyaluronic acid (HA) gel injected into the body and border of the lips. HA is a sugar molecule your body already makes, and the fillers we use contain lidocaine for comfort.
- What changes: actual volume and shape. Fuller lips, a more defined border, corrected asymmetry.
- Onset: visible immediately, final result at about 2 weeks once swelling settles.
- Duration: roughly 6-12 months in the lips.
- Downtime: expect swelling that peaks at 24-48 hours and settles over 3-7 days, with possible light bruising.
- Reversible: yes. HA filler can be dissolved with hyaluronidase if needed.

Which Should You Try First?
Try a lip flip first if:
- You like your lip shape and just want your upper lip to show a little more when you smile
- Your upper lip disappears or thins when you smile
- You are curious about lips but not ready to commit to filler
- You cannot have any downtime right now
Go straight to filler if:
- You want lips that are noticeably fuller, at rest and in photos
- You have lost lip volume with age and want it restored
- You want to correct asymmetry or define a thin border
- A flip's 6-8 week duration would frustrate you
Can You Combine Them?
Yes, and many patients do. Filler provides the volume and shape; a flip adds a small lift to the upper lip when you smile. They can be done at the same visit or staggered. At your consultation I will tell you honestly whether one, both, or neither is the right call for your face.

The Honest Caveat
A lip flip is subtle. If you are expecting the change a syringe of filler gives, you will be disappointed. Half of my job at a lip consultation is matching the treatment to the expectation. Sometimes that means telling a patient who asked for a flip that what she is describing is filler, or telling a patient who asked for filler that a flip will get her there for a fraction of the cost.
